Appendix 5348. Required quarterly reports.

(a)

Each collector shall provide the city within thirty (30) days following the end of every calendar quarter separate tonnage reports broken down by month for each collection route, which include the following:

(1)

Total tonnage of solid waste removed from the city for the reporting month, categorized by commercial and residential, and also by solid waste versus residual.

(2)

Total tonnage of recyclables collected from the city for the reporting month, categorized by commercial and residential.

(3)

Number of accounts served, categorized by commercial and residential.

(4)

Number of complaints and missed collections

(5)

Receipts from sale of recyclables.

(6)

The name, address and telephone number of each solid waste disposal and/or recycling facility used by the collector during the reporting period.

(7)

All information required by the city's recycling and resource recovery program.

(8)

Any other information deemed necessary by the city manager.

(b)

Each report shall be signed by an officer of the collector.

(c)

The timely filing of complete monthly reports satisfactory to the city manager is a condition of any permit or franchise awarded by the city.
